Staff Personal Details Form的某些项目可以拒绝填写吗？

刚加入新公司，HR给了我一份Staff Personal Details Form, 其中有几项让我觉得很不舒服
1）Previous Company Total Salary Payment
2) Previous Company Total KWSP Contributions
3) Previous Company Total PCB Contributions
4）spouse的资料里，有要求Spouse 的Income Tax No/Branch
最后端需要Staff Declaration & Signature, 要tick以下2个declaration & sign:
1) I hereby confirm that the details provided herein are to my knowledge true and correct. I also understand that the company shall not be liable in the event that any of the above is found to be untrue or incorrect and/or I am liable for disciplinary action as the company deems fit. It shall be my responsibility to notify Human Resourse Dept. on any changes in the above throughout the year.
2) I confirm that there are no changes on my personal details. I also understand the company shall not be liable in the event that the above is found to be untrue or incorrect and/or I am liable for disciplinary action as the company deems fit. I shall be my responsibility to notify Human Resource Dept. on any changes in the above throughout the year.
各位大大有面对或填写过这样的资料吗？我认为这4项不关新公司的事，不想填写，如果拒绝填写会有什么后果？如果不填，会against到这一句“I am liable for disciplinary action as the company deems fit"吗？
很困扰，有经验的大大或HR的大大教教我怎么做？ .gif) |

3，4应该没问题，是关系到你income tax的事。1，2就好象 |

根据我公司hr说，如果你跟你的spouse是同一个file，需要缴付的income tax比较少 |

declaration:
I hereby confirm the information stated above is true
Failing which I understand that penalty / legal proceeding may be taken against me by the inland revenue board

谢谢ygverygeng的info, 银行是比较严，但我的公司只是普通公司罢了
有哪位HR的大大可以告知为什么HR需要这些资料？
1）Previous Company Total Salary Payment
2) Previous Company Total KWSP Contributions |

我就曾经看过公司的 job application form 连家人,兄弟姐妹的工作,公司, 薪金和学历都要求写出来的. 个人hobbies也要知道 |

每家公司不同的行政方式。怎样告诉你？ 我做了两家公司，第一家只是基本资料，第二家时就什么都没 ...

1和2可以拒绝填。你新公司没有那个权力要求知道你的财政资料，如果新公司的HR问你为什么不填，你就说，1和2是很private的东西，我不需要公开给你知道。
或者你可以酱紫写：
1和2酱写： not applicable. |
